About Evidence-Based Programs
Evidence-Based Programs are interventions based on evidence that is generated by scientific studies published in peer-reviewed journals. Model Programs are a type of evidence-based program. See the Model Programs section of our site for more information and examples.
Evidence-Based Programs at AoA
The U.S. Administration on Aging has awarded funding to 24 states under its Evidence-Based Disease and Disability Prevention Program. Under this initiative, AoA requires each participating state to implement the Stanford University Chronic Disease Self-Management Program (CDSMP), but also gives each state the option to select another evidence-based program which helps reduce chronic disease in its senior population. The Center for Healthy Aging web site was created to serve and support those grantees in implementing evidence-based programs. For more information on the funded projects, see the profiles below.
